Respected Sir/Madam,

With due respect, I would like to inform you that I couldn’t take my (Subject name) Exam on (date: DD/MM/YY) as I was fall in car/bike accident. I was completely prepared for the paper but on the last moment, I fall in this trouble.  I was involved in a car accident this morning, and thankfully no one was seriously hurt. When I got home I tried to study for my exam tomorrow, but I don’t have the energy, or concentration to work on it.
